What we do for Lexington clients

Transcription, human-verified or AI-only

Upload audio or video, or dictate straight into the browser. You choose per job whether a person checks the transcript against the recording before you get it, and the finished document records which mode produced it.

Dictate a form and get the form back

Upload a blank form, talk through the answers, and we write them into that same document in its own layout - not a summary of what you said. Every extracted value carries a confidence score and the quote behind it.

A record that survives an audit

When someone approves a document we store who they were, a SHA-256 digest of exactly the content they saw, the version, and the time - plus how long elapsed between the transcript arriving and the signature.

Turnaround you pick per job

Human-verified orders carry a minimum: $25 for audio and documents, $30 for video. AI-only orders have no minimum. Recordings with heavy background noise, crosstalk, or strong accents are quoted at the difficult-audio rate, from $2.25 per minute. We tell you before work starts, never after.

How we work in Lexington

Local transcriptionists

We engage transcriptionists based here, so a reviewer working on a Lexington practice's files knows the local institutions and terminology that trip an automated system.

Partner practices

We work with legal practices in Kentucky to serve their clients, which is how we cover this market without asking anyone to send files to another state.

We come to you

For onboarding we would usually rather visit your office than have you visit ours. The Lexington office is available by appointment when meeting here suits you better.

Request an appointment in Lexington

Tell us what you need and we will agree a time. We are happy to come to your office instead.

We will only use this to answer your request. No account needed, and you can also start a transcript right now without talking to anyone.

What Kentucky law requires

Medical records

Kentucky requires a hospital to retain patient records for five years following discharge, and for a minor for five years or until the patient turns twenty-one, whichever is longer. There is no state requirement for a private practice at all, which is the more important fact for most buyers: a Kentucky physician office sets its own retention policy, bounded in practice by HIPAA's six-year documentation requirement and by the state limitation period for a malpractice claim. The absence of a rule is not permission to destroy records early - it means the exposure is decided by litigation risk rather than by a licensure penalty.

No state rule for private practice. Retention is governed by HIPAA and litigation exposure, not licensure.

  • Kentucky provides no retention requirement for private practices

The regulatory information on this page is general background compiled from public primary sources, not legal or compliance advice. Requirements change and vary by jurisdiction and by court. Verify current rules with the relevant authority or your own counsel before relying on them.
